Prior to completing your purchase (or on the same day of purchase), follow the link on this page to complete a health survey. If you qualify, Truemed will provide a Letter of Medical Necessity, which you can use when submitting your expense for HSA/FSA reimbursement.

- COMPLETE HEALTH ASSESSMENT: Complete a private health survey. An independent licensed practitioner will review your answers to determine eligibility. If approved, you will receive a Letter of Medical Necessity (LMN). The survey must be taken on the same day as your purchase.
- SUBMIT FOR HSA/FSA REIMBURSEMENT: Reimbursement timing is determined by your plan administrator.

1 on 1 personal training and 2 on 1 personal training (when paid "per person") are eligible. Any service purchased at Volition Fitness must be paid for and used by the client to which the LMN was issued.
